Consultation Policy

All new clients must complete an intake form prior to their consultation

Consultations are required before any cranial prosthetic is created or custom ordered

Medical documentation (if applicable) should be submitted before or at the time of consultation

Consultations may include scalp assessment, measurements, and design planning
Medical Documentation Requirements
For clients seeking insurance reimbursement or medically necessary prosthetics:

A Prescription or Letter of Medical Necessity may be required

Diagnosis must be provided by a licensed healthcare professional

We may coordinate with your physician to obtain required documentation (with signed HIPAA authorization)
Payment Policy

A deposit is required to begin all custom cranial prosthetic orders

Custom units are non-refundable once production has started

Accepted payment methods: Visa, Mastercard, Amex, Discover, Afterpay Klarna, Google pay, Apple Pay, PayPal, Affirm

Insurance reimbursement is not guaranteed and is the responsibility of the client unless otherwise specified

Production timelines vary and will be discussed during consultation

Fittings & Add on Services

Request virtual or in person fitting appointment

Cut & Style services begin at $75

Color, Cut & Style services begin at $250

Additional chemical services like bleaching or perm available by request
Hygiene & Safety Policy

Services will not be performed on clients with open wounds, infections, or contagious scalp conditions without medical clearance

All tools and materials are sanitized according to professional and health standards

Clients must disclose any sensitivities or allergies prior to fitting
Cancellation & Rescheduling Policy

48-hour notice is required for cancellations or rescheduling

Late cancellations may result in a fee

No-shows may require a deposit to rebook
Privacy & Confidentiality

Returns & Exchanges
